Linkage Project
Developed by the OHSU Oregon Office on Disability and Health (OODH)
Get Connected to Preventive Care That Works For You with The Linkage Project
The Linkage Project Team from OODH helps in three (3) ways:
1. Identify Needs
They ask people in the project questions about their preventive health needs.
2. Educate
They help people learn about healthy behaviors, going to the doctor, going to the dentist, getting screening tests, and getting mental health counseling.
3. Find Health Supports
They help people find preventive health care and other health supports in their community.
